I understand and acknowledge that the activity to be engaged in through my rental of an inflatable interactive amusement device and/or
a concession device, brings with it the both known and unanticipated risks to myself, my quests, and to my invitees. Those risks include,
but are not limited to; falling, slipping, crashing, and colliding, and could result in injuries, illness, disease, emotional distress, death,
and/or property damage to myself, my quests, to my invitees of third parties. I being aware that this activity entails risks or injuries to
myself and a risk or injury to spectators or third parties as a result of my actions, expressly agree and promise to accept and assume all
responsibility and risk for injury, death, illness or disease, or damage to myself or to my property arising from participation in this
activity. I also agree to pay for any damages caused to others including attorneys' fees and costs if they are injured or otherwise damaged
due to any negligent actions. My participation in this activity is purely voluntary; no one is forcing me to participate, and I elect to
participate in spite of the known and unknown risks. I for myself and on behalf of my heirs, assigns, personal representatives, next of kin
and any minor children or adults I am otherwise responsible for, voluntarily release, indemnify, hold harmless and discharge ROCK STAR
JUMPERS in the defense of any such liability claim, demand, action or right of action.
I acknowledge that I have the adequate homeowners insurance, tenant insurance, or other liability insurance to cover any bodily injury
which might occur to myself, my quests, or my invitees from the use of the equipment I am renting and I agree to bear the cost of such
injuries myself. I agree to bear all cost to the rental equipment that is stolen, lost and/or damaged and the customer is required to
replace, and/or repair the equipment rented and will be in the property of ROCK STAR JUMPERS within 30 days of loss/damage and
customer will pay any legal fees to ROCK STAR JUMPERS.
Rock Star Jumpers will not be held responsible for underground utilities or sprinkler systems when staking down equipment. If you are
not sure about these underground utilities for your set-up location, please contact the utility companies or JULIE at 1-800-892-0123.
I agree to have an adult supervise both the equipment and its use at all times while equipment is in the possession of the customer. A
set of rules and directions are displayed on the unit and the driver setting up the unit will also explain the safety rules which I agree to
follow and utilize at all times during use and operation of the unit. The customer understands that no operator has been left with the
unit(s) and is solely responsible for the correct and safe operation of this equipment. Customer voluntarily agrees to keep and maintain
all safety rules that have been explained by the driver and rules listed on the unit.
There must be a clear path to where the unit will be set up. Minimum 4 foot wide clearance. If there is not a clear path, the driver will NOT
set up the unit. Customer must move all items blocking path prior to delivery.
Driver's pick-up time is approximate. Customer is responsible for all equipment until picked up by the driver.
The unit's electrical cord MUST be connected to a GFI electrical outlet at all times.
Unit's shall be inflated at ALL times until the driver picks up the unit so that the unit can be inspected.
All equipment must be located at the address listed below during the rental period.
The unit(s) will be inspected upon delivery and before removal for cleanliness, damage, and functionality. If the unit requires cleaning
by the driver, there will be an additional cleaning charge of $50.00 minimum, payable to the driver. ABSOLUTELY NO "SILLY STRING" to be
in, on, or around the Equipment, as it will damage the vinyl. Any damage to the unit will be charged to the customer at the rate of $50.00
minimum per incident.
Problems do arise at times and if there is any problem, the customer will contact ROCK STAR JUMPERS so that an employee can be sent out
to correct the problem.
Our weather policy is that if it is raining, lightning, or strong winds, we have the right to cancel the delivery. If during the rental period, it
is raining, lightning, or strong winds, we have the right to pick up the equipment and no refund will be given.
I acknowledge and certify that I have had sufficient opportunity to read the entire Rental Agreement and Acknowledgement of Risks, that
I understand its content and that I freely, intelligently, and without duress of any kind agree to it by its terms. I further warrant and
represent that I am either the customer named below, or I am authorized and empowered to accept delivery of the equipment and to sign
this agreement on their behalf. Further, I agree that I am binding myself personally as an additional party to all of the terms and condit-
ions of this agreement. I also agree that I am over 18 years of age as proved by acceptable identification. Identification must be
presented at the time of delivery.
